Do you ever wonder just how safe those rides are at local carnivals? It appears a pair of improper bolts caused an amusement park ride to break apart at a church fair in Shrewsbury, Massachusetts.

Sunday's accident killed one person and injured two others. The accident happened on a ride called The Sizzler.

The man who died was thrown from the ride and hit his head on the pavement. Two others were hurt. All three were from a state-run home for the mentally retarded.

Eleven Sizzler rides in Massachusettes have had their permits suspended while the state investigates.
